The following forms are utilised to ensure that we hold all the correct information on your child to ensure that we meet any of their individual requirements and that a family member can be contacted in case of an emergency. We also want to work with you to ensure that we are aware of your consent as parents to share certain information; and that all parties involved are aware of each other's responsibilities and duties to work together in the best way.

It is essential that you inform the school office of any changes in address, contact numbers or family circumstances that may affect contact arrangements. Any confidential matters can be discussed with the class teacher or a member of senior management.

The parental consent form is valid for the duration that your child attends Boothstown Primary School. Parents/Carers have the right to amend/ withdraw their consent at any time. You can do this by emailing boothstownmethodist.primaryschool@salford.gov.uk.

Parental Consent In line with Government guidelines we require parental consent for your child to take part in certain activities within school. Please read through each statement and confirm which you agree to your child participating in, whilst at Boothstown Methodist Primary School.

Medical Need 2022/23 It is important that we are aware of any medical condition/s that your child may have- asthma, allergies, heart murmur etc. This information will enable us to help your child avoid illness, identify symptoms of impending illness or how to treat any such conditions. Please complete the information below so that we can check the details and send home the relevant forms to request further information, depending on the condition and the information we already hold. This information will remain confidential and any sharing of this information amongst staff or pupils to ensure your child's safety will be discussed with you. It is essential you still complete this form, even if you think you have provided this information to the school previously.

Data Collection Form 2022/23 In previous years, at the start of the new academic year, we have sent out pupil data forms for parents/ carers to update their personal details (i.e. address, telephone numbers and emergency contacts). Due to GDPR guidelines, we will no longer do this for existing pupils. We therefore ask that you complete this online data collection form. Throughout the year please ensure the school office is informed of any changes to your personal details by emailing boothstownmethodist.primaryschool@salford.gov.uk. This is extremely important, especially if parents/ carers need to be contacted in an emergency. Thank you.
