Agreements

The following forms are utilised to ensure that we hold all the correct information on your child to ensure that we meet any of their individual requirements and that a family member can be contacted in case of an emergency.  We also want to work with you to ensure that we are aware of your consent as parents to share certain information; and that all parties involved are aware of each other's responsibilities and duties to work together in the best way.  

 

It is essential that you inform the school office of any changes in address, contact numbers or family circumstances that may affect contact arrangements.  Any confidential matters can be discussed with the class teacher or a member of senior management.

 

The parental consent form is valid for the duration that your child attends Boothstown Primary School. Parents/Carers have the right to amend/ withdraw their consent at any time. You can do this by emailing boothstownmethodist.primaryschool@salford.gov.uk.
